Get Connected with our Senior Connection Program

Lake Health offers Senior Connection, a membership program for men and women age 60 and over. Membership is FREE. Senior Connection is designed to be fun and informative.

Our Senior Connection Lunch & Learn programs cover a variety of topics and issues related to seniors, health and wellness, lifestyles, and the latest medical news. Registration is a must at these popular luncheons, which cost only $2 for members.

STEP- Senior Total Evaluation Program

Through our STEP program, we'll evaluate at-risk seniors in their homes, using a multidisciplinary team. We'll tailor the evaluation to the individual’s needs, focusing on physical, psychosocial, environmental, and safety status. After the evaluation we'll provide a written report to the patient, family, and physician.

Center for Geriatric Psychiatry

The Center for Geriatric Psychiatry offers hope to those 65 years of age and older who are experiencing serious emotional difficulties. We extend a helping hand through an admission assessment and individualized treatment plan. Inpatient services focus on symptom relief and adaptive skill development to increase independence and mobility. This involves identification of capabilities, acceptance of limitations, and development of appropriate medication regimen. Services are provided by a licensed, professional staff comprised of psychiatrists, physicians, nurses, social workers, and therapists.

Located within West Medical Center in Willoughby, The Center for Geriatric Psychiatry treats a variety of medical problems in conjunction with a primary psychiatric diagnosis. The location also permits easy access to neurological and other diagnostic procedures that help identify the causes of a patient's difficulties. This integrated approach enables the clinical team to determine the best course of treatment.
