All physicals/examinations include the following:
- Visual acuity and vital signs, including blood pressure, urine dip test, height, weight
- Occupational/medical history
- Cardiovascular evaluation
- Musculoskeletal examination
- Respiratory system evaluation
- Gastrointestinal examination
- Examination of head, eyes, ears, nose, and throat
- Skin and lymphatic examination
- Neurological evaluation
Fit-for-Duty examinations will utilize the parameters of the of the job tasks to determine if the potential employee meets the necessary physical requirements to perform the job. Employer will need to provide functional job descriptions to Lake Health prior to the scheduled physical.
Regulatory or Compliance examinations are to determine medical baselines for the employee to utilize with annual monitoring program (OSHA, Department of Transportation, and/or your internal wellness program).
Employee Well-Care physical can assist your internal voluntary wellness program initiatives to establish baselines and monitoring for improvements in health risk factors. Lake Health offers health risk questionnaires (HRA) with health risk reports for the individual.
Additional services may include:
- Audiometric testing
- Chest x-rays
- Laboratory testing
- Medical monitoring service
- Pulmonary testing (spirometry)
- OSHA respirator questionnaire
- Vision testing for specific standards.
