Health care has changed. Advanced technology and managed care can help you get out of the hospital and back in your home faster than ever. True, you're much more comfortable at home, but sometimes you need help.

If you need medical assistance at home to recover from an injury, illness, or surgery, you can rely on Lake Health Home Care Services, even if you haven't spent any time in the hospital.

A Team Approach

Maybe you've been diagnosed with diabetes and are confused about your new routine. You might have suffered an injury and need help changing your dressing. Or you could be recovering from a stroke, feeling overwhelmed by its impact.

Whether you need short-term or on-going care, I.V. therapy or physical therapy, you can count on the professional health care team at Lake Health Home Care Services. We'll evaluate your condition, teach you the best way to care for yourself, and practice with you until you are comfortable providing your own care.

We use a team approach, and we treat you as an important member of the team. Depending on your needs, members of the team might include nurses, home care aides, therapists, and social workers. The team always includes your physician.

STEP (Senior Total Evaluation Program)

We'll evaluate at-risk seniors in their homes, using a multidisciplinary team. We'll tailor the evaluation to the individual's needs, focusing on physical, psychosocial, environmental, and safety status. After the evaluation we'll provide a written report to the patient, family, and physician.

Getting started

When your doctor recommends home health care, ask for a referral to Lake Health Home Care Services. After your doctor refers you to us, we'll call you to set up an evaluation. Together, we'll develop a plan of care that meets your needs. While we're caring for you, we'll work closely with your doctor and keep him or her informed about your progress.

A variety of insurance plans cover home health care, including Lake Health Home Care Services. Whether you have private insurance, Medicare/Medicaid, or are self-pay, we provide care based on your doctor's direction. We'll act as your advocate to help you sort through any insurance confusion.
