Individualized Care
Working with your physician and based on your symptoms, the staff at the Heart Failure Clinic will develop an individualized treatment plan that’s right for you. At each visit, our nurse practitioner will monitor and assess your medications, blood pressure, weight, and breathing for any changes in your condition. If there is a change in your condition, our nurse practitioner will adjust your medication or treatment plan.
Ongoing Support and Education
During your visit, our nurse practitioner can help you understand your disease by explaining the symptoms and physiological effects of your condition, offer helpful nutrition counseling, and answer any questions. The clinic staff also provides access to a valuable support network, including dietitians, social workers, and physical and respiratory therapists. The staff can even coordinate your physician and therapy visits too.
