- Close monitoring of disease and renal status
- Fluid, electrolyte, anemia, and bone disease management
- Education about disease signs and symptoms
- Education on diet, exercise, and medications
- Support for you and your family.
For an appointment
To schedule an appointment with the Chronic Kidney Disease Clinic, you will need a physician referral. Bring that and a list of your medications to your first visit. The Clinic is located at West Medical Center and TriPoint Medical Center. New patients to the clinic must stop at the Registration Desk on the first floor of the hospital before their first appointment. Please arrive 15 minutes early.
