Chronic Kidney Disease Clinic

Chronic kidney disease is progressive and serious. But with proper care and education of patients, their families, and their physicians, kidney disease can be managed and its progression slowed.

Effective disease management will help you delay the need for dialysis, avoid hospitalization, and allow for an improved quality of life.

If you have kidney disease, Lake Health’s Chronic Kidney Disease Clinic can help. Our nurse practitioner and dietitian will offer ongoing education and treatment, including:

  • Close monitoring of disease and renal status
  • Fluid, electrolyte, anemia, and bone disease management
  • Education about disease signs and symptoms
  • Education on diet, exercise, and medications
  • Support for you and your family.

For an appointment

To schedule an appointment with the Chronic Kidney Disease Clinic, you will need a physician referral. Bring that and a list of your medications to your first visit. The Clinic is located at West Medical Center and TriPoint Medical Center. New patients to the clinic must stop at the Registration Desk on the first floor of the hospital before their first appointment. Please arrive 15 minutes early.
